Registration Information

Here are the requirements for registration. For questions regarding new student registration, call Roscoe Middle School at 815.623.1884 between 8:30 - 11:00 am, Monday through Friday until July 8th.

 

In-school registration

Please go to Ledgewood or Stone Creek Elementary Schools for grades K-3,
Kinnikinnick School for grades 4-5 and Roscoe Middle School for grades 6-8.
Offices are open from 8-3.

REGISTRATION WILL ONLY BE COMPLETED WITH ALL PAPERWORK.

 

Proof needed for registration

  • Two (2) proofs of residency (e.g. closing papers, lease agreement, utility bills, anything with name and new address, change of address form from the post office, owner occupied exemption form.) Drivers Licenses will not be accepted.
  • Certified copy of child's birth certificate
  • Name, address and phone number of child's previous school.
  • Current Immunization and current physical examination records. If new to Illinois, you must submit an Illinois State Physical Form.
  • IEP and/or 504 paperwork, if applicable.

Payment

We are continuing our mail-in procedure for student registration, payment of fees, and residency verification. Fees may be paid by check or credit card (MasterCard, Discover and American Express) using Illinois E-Pay. There is a convenience charge collected by the credit card companies for this service.

 

Education of Homeless Children

Each child of a homeless individual and each homeless youth has equal access to the same free, appropriate public education. A “homeless child” is defined as provided in the McKinney Homeless Assistance Act and State law. The Superintendent shall act as a Liaison for Homeless Children to coordinate this policy’s implementation. If you have questions, regarding your child, please contact 815-623-2837.
