We are continuing our mail-in procedure for student registration, payment of fees, and residency verification. Fees may be paid by check or credit card (MasterCard, Discover and American Express) using Illinois E-Pay. There is a convenience charge collected by the credit card companies for this service. Illinois Child Health Examination (Physical) Form required before school entry into Pre-school, Kindergarten, 6th grade and 9th grade. Also required for students new to the district.

Illinois Dental Examination Form required for Kindergarten, 2nd and 6th grades, due May 15th of each required year.

Illinois Dental Waiver Form

Illinois Vision Examination Form required for Kindergarten entry and all students entering an Illinois school for the first time.

Illinois Vision Examination Waiver Form

IESA/IHSA Sports Examination (Physical) Form required for tryouts and participation in all school sports, must be updated annually.

Epinephrine Action Plan and Authorization for students with life threatening allergies-must be signed by physician and parent, must be updated annually.

Medication Authorization Form for students who need medication while at school-must be signed by physician and parent, must be updated annually.  Read a letter from Julie Cropp, Principal at RMS.
